Credit Retrieval
Please use the buttons below to claim your continuing education credits and certificate for attending.
CONTINUING MEDICAL EDUCATION (CME), CONTINUING
NURSING EDUCATION (CNE), AND ABIM MOC CREDIT
Step 1: Visit ACHLcme.org. Click on “Use Certificate Code” on the top right.
Step 2: Enter the Certificate Code: 56901243.
Step 3: Register or log into your account.
Step 4: Complete the evaluation and post-survey forms. Once complete, a
certificate will be emailed to you and will be available for immediate download.
Note: Please check spam/junk folders if email is not received.
A form must be completed each day to receive ASRT CE Credit
Perfusion Category One CEU Credits must be claimed by completing a form at the registration desk at the Houston Shock Symposium.
*Guidelines require that learners in CME/CNE activities be made aware of all relevant financial relationships affecting the presentation of the faculty member. ACHL/Memorial Hermann Health System Provider ensures that all faculty and staff involved in the planning, development, and implementation of ACHL/ Memorial Hermann Health System Provider activities disclose financial relationships held within 24 months and that relevant financial relationships are mitigated before a CME/CNE activity occurs. Complete information will be provided to learners prior to the start of the educational activity.
Instructions:
There will be a different form for each day of the symposium that will need to be completed and submitted in order to receive full credit.
Following the completion of the Houston Shock Symposium, look for an email with an evaluation and link to obtain a copy of your completion certificate. Please retain a copy of the certificate for your professional records.
